背景
皮膚および粘膜における侵襲的デバイスの使用および損傷は、感染症に対する感受性を高める。看護師は頻繁に無菌操作（PRA）を実施するが、様々な課題や実践における不当なばらつきが報告されている。
目的
看護師を対象に、その経験、PRA を実施する上で必要とされる情報や支援について認識されているギャップについて探索する。
方法
2021 年 9 月から 2022 年 1月にわたり、英国国民保健サービス（NHS）の看護師 20 名に対して、グラウンデッド・セオリーで採用されているサンプリングおよびデータ収集のアプローチを用いて質的面接を実施した。
結果
回答者は、様々な臨床環境で雇用されていた。回答者は、手術室以外では、無菌状態を維持しようとする試みは不可避的に妨げられるが、汚染されやすい場所の汚染リスクを抑えるために、状況にかかわらずまだできることが多くあると考えていた。不適切な実践が報告され、回答者らはルーチンの手技（慢性創傷のドレッシング、血管内留置ラインの操作など）を実行するために無菌状態が必要であるかどうかについては確信を持っていなかった。問題の原因として考えられることは、不十分な看護教育、継続的な専門的能力開発への不十分なアクセス、および経験の浅い看護師や医療スタッフの不注意が挙げられた。回答者らは、PRA を実施するためのより詳細なガイドラインを求めていた。経験の長い看護師は、手技の実施は状況にかかわらず同じ方法でなされることを望んでいた。PRA を実施した看護師は一致して、ガイドラインは柔軟であるべきであると示唆した。
結論
PRA についての知識を提供するための詳細な情報を示すガイドライン、臨床的向上の機会へのより良いアクセス、および登録前の看護教育の改善が必要である。現在の標準を満たすため、ガイドラインの作成には認められた方法論を採用すべきである。看護学生に対しては、実際の患者と接する前にシミュレーション実習において、状況に応じて PRA を実施・調整するために必要となる知識および技術を指導すべきである。

英国での質的面接研究。無菌操作（PRA）は感染を防ぐための特定の手順（さまざまな侵襲デバイスの挿入と管理、創傷管理など）の一部を形成し、独立した手順として見過ごされた可能性から、看護師が行う無菌操作の標準化や質の高いガイドラインや教育の必要性について、改めて分析されている。
