背景

マイコバクテリウム・アブセサス(Mycobacterium abscessus)群は、嚢胞性線維症(CF)やその他の化膿性肺疾患の患者に感染症を引き起こすことのある環境微生物である。同じ医療センターを受診している CF 患者において、M. abscessus のヒト‐ヒト間空気伝播の可能性がある。紫外線 C 波(UVC)は、結核菌(Mycobacterium tuberculosis)を屋内で制御するために用いられているが。空中 M. abscessus に対する UVC について評価した研究はない。

目的

一定範囲の照度の UVC 照射により、UVC 非照射条件下と比べて空中 M. abscessus の不活化が増大するかどうかを明らかにすること。

方法

容量 400 L の回転式ドラムサンプラー内に設置したメッシュ式ネブライザーを振動させて M. abscessus を生成し、次いで波長 265 nm で一定範囲の照度の UVC を発光する発光ダイオード(LED)に曝露した。6 段階のアンダーセンカスケードインパクターを用いてエアロゾルを採取した。標準的な微生物学プロトコールを用いて M. abscessus を計数し、次いでこれらの結果により UVC 照度の有効性を定量した(3 回実施)。UVC の有効性を、UVC を用いた場合と用いない場合における不活化の差を用いて推定した。

結果

16 回の試験を実施し、UVC の照度は 276 ~ 1,104 µW/cm2 の範囲であった。UVC の有効性の平均値(±SD)は、47.1%(±13.4)~ 83.6%(±3.3)の範囲であった。UVC により、評価を行ったすべての照度における自然減少よりも有意に大量の M. abscessus の不活化が得られた(すべてについて P ≦ 0.045)。病院環境を模した屋内モデルを用いた場合、上記の試験に用いた範囲の照度の UVC により、患者およびスタッフがいる臨床現場において安全に照射できると推定された。

結論

本研究は、エアロゾル化された M. abscessus に対して UVC による不活化が可能であるという実験的 in-vitro エビデンスを提供している。
