背景

2016 年 9 月から 2020 年 11 月の間に、イングランド北部の 3 次病院 1 施設の血液内科患者において、17 症例の難治性耐性緑膿菌(Pseudomonas aeruginosa)が報告された。

目的

難治性耐性緑膿菌感染症と臨床的介入、患者の移動、抗菌薬使用、併存疾患との間の関連性を検討するため、後向き症例対照研究を実施した。

方法

症例は、陽性検体が認められる前に入院していた難治性耐性緑膿菌アウトブレイク株の保菌または感染患者とした。診療記録から曝露を抽出し、条件付きロジスティック回帰を用いて症例を対照と比較した。環境および微生物学的調査も実施した。

結果

症例 17 例と対照例 51 例を対象とした。最終モデルには年齢(> 65 歳、補正オッズ比[aOR]6.85、P = 0.232)、性別(aOR 0.60、P = 0.688)、移植チームの管理下の入院(aOR 14.27、P = 0.43)、シプロフロキサシンの使用(aOR 102.13、P = 0.030)が含まれた。本調査では症例から症例への伝播または感染源は示されなかったが、共通の環境感染源の可能性は高かった。

結論

本研究により、血液内科患者においてフルオロキノロンの使用は難治性耐性緑膿菌の独立したリスク因子の 1 つであることが分かった。標準的な感染制御介入に加えて、抗菌薬適正使用支援とフルオロキノロンの予防投与の見直しを緑膿菌アウトブレイク調査の一環として検討すべきである。

監訳者コメント

難治性耐性緑膿菌はすべてのβラクタム系薬およびフルオロキノロン系薬に耐性の緑膿菌と定義される。英国の国立衛生研究所(NICE)のガイダンスでは好中球減少患者に対して一次予防としてフルオロキノロンが推奨されている。一方で、フルオロキノロンを予防的に使用することにより、耐性菌が定着または感染する可能性が高くなることがわかっており、本研究もそれを示唆する結果となっている。フルオロキノロン予防による罹患率と死亡率の低下の利点と、患者のリスクおよび局所の抗菌薬耐性への影響とのバランスをとるためにさらなるデータの蓄積が必要である。
