背景
化学的または物理的(蒸気滅菌など)な不活性化に対して、プリオンは抵抗性が異なることで知られている。不活性化効果が不十分な場合、汚染した手術器具によるクロイツフェルト・ヤコブ病の医原性伝播のリスクを患者にもたらす。
目的
スクレイピー病原体に対してもっとも確立したプリオン不活性化法が立証されたが、それらは概してヒトプリオンよりも耐熱性が低いことが確認された。したがって、蒸気滅菌によるクロイツフェルト・ヤコブ病プリオンの動態に関する知見の隔たりを埋めるべきであり、現行のガイドラインを適宜見直す必要がある。
方法
広く推奨されている 134°C での蒸気滅菌によるプリオンの不活性化について、異常折りたたみ蛋白反復増幅(PMCA)法による残存プリオンシーディング活性の分析により、複数の保持時間で評価した。
結果
スクレイピー 263K 株は、134°C、1.5 分後にシーディング活性を示さない耐熱性最小のプリオン株であることが確認された一方で、変異型クロイツフェルト・ヤコブ病は、蒸気滅菌 18 分後でもかなりのシーディング活性を示す最も強固なプリオン株であった。孤発性クロイツフェルト・ヤコブ病のサブタイプ VV2 は、134°C で、3 分後に残存シーディング活性を示したが、5 分後に活性は検出できなかった。
結論
リスクの高い組織に接着した手術器具のルーチンでの再処理において、以前推奨された 134°C、5 分間の検証済み蒸気滅菌は、クロイツフェルト・ヤコブ病の病原体のシーディング活性をかなり低減できる。ただし、滅菌前に化学的な固定消毒法を実施せず、十分な洗浄により表面上の蛋白負荷が器具当たり 100 μg 未満に減少した場合に限る。
監訳者コメント:
検証済み 134°C プログラムの使用の蒸気滅菌によるクロイツフェルト・ヤコブ病のプリオンシーディング活性低下は、これまでの実績から受け入れられやすい簡便な対処法である。
