背景

ニュージーランドにおける医療関連感染症(HAI)に関する現在のデータはない。

目的

ニュージーランドの公立病院の成人における HAI の疫学、医療機器の使用頻度、および HAI の微生物学的特徴を明らかにすること。

方法

点有病率調査。調査者は、18 歳以上の患者について、欧州疾病予防管理センター(ECDC)による HAI の定義を用いてレビューした。医療機器の使用および HAI の微生物学的特徴を記録した。

結果

全体で、患者 5,468 例を調査し、361 例(6.6%)に 423 件の HAI が認められた(患者 100 例あたり HAI 7.7 件)。最も多かった HAI は、手術部位感染症(N = 104、25%)、尿路感染症(N = 80、19%)、肺炎(N = 75、18%)および血流感染症(N = 55、13%)であった。全体で、3,585 例(66%)で 1 個以上の医療機器が使用されており、2,922 例(53%)で末梢静脈カテーテルが留置されていた。HAI の69 件(16%)はデバイス関連であった。多変量解析では、HAIの独立リスク因子には、末梢(オッズ比[OR]2.0)または中心(OR 5.7)静脈カテーテルの存在、ならびに診療サービスの種類が含まれた。HAI の発生率は、外科病棟患者(OR 1.8)、集中治療室患者(OR 2.6)およびリハビリテーション病棟/高齢者病棟患者(OR 2.4)では、一般内科病棟患者よりも高かった(すべてのグループについて P ≦ 0.01)。全体で、301 の微生物が同定された。クロストリジオイデス・ディフィシル(Clostridioides difficile)感染症は頻度が低く、すべての HAI の 1.7%であった。分離株 42 株(14%)は薬物耐性株であり、多くは腸内細菌目細菌であった(N = 33、79%)。

結論

本研究により、ニュージーランドにおいて最も頻度の高い HAI とそのリスク因子が確立された。高頻度の医療機器使用から、効果が証明されたマルチモーダルな予防介入を確実に導入することの必要性が強調される。しかし、HAI のうち医療機器または手術に関連するものは半数未満であったため、感染症の負担を低減するためには別の介入戦略が必要とされるだろう。
