背景
クロストリジオイデス・ディフィシル（Clostridioides difficile）感染症（CDI）の診断および臨床管理に関する国際ガイドラインへの認識および遵守については知られていない。
目的
2018 年から 2019 年の欧州全域において、CDI の診断および臨床管理に関する推奨戦略への認識および遵守について比較すること。
方法
2018 年から 2019 年に、欧州 12 か国の病院および関連する地域診療所がオンライン調査に回答して、CDI のサーベイランス、予防、診断、および治療に関する各々の実践について報告した。回答は、病院 105 施設および地域一般診療医 39 名から得られた。
結果
11 か国の病院は国内サーベイランス制度に参加していると報告したのに対して、6 か国は国際サーベイランス制度に参加していた。欧州臨床微生物学・感染症学会（European Society of Clinical Microbiology and Infectious Diseases；ESCMID）が推奨する CDI 検査法は、82％の病院（86/105）で使用されていたが、最も高い CDI 発生率を報告した国々では推奨されていない検査が使用されていた。75％以上の病院（80/105）は、本調査の実施時点で直近の欧州 CDI 治療ガイドラインについて認識していたのに対し、調査した一般診療医で認識していたのはわずか 26％（10/39）であった。しかし、15％以下の病院（16/105）が再発 CDI 症例に対して、推奨されていないメトロニダゾールを使用していると報告し、これらの病院は CDI 治療ガイドラインに対する認識が低い国々に所在していた。CDI が疑われる症例に対して接触隔離予防策を採用していたのは、わずか 37％の病院（39/105）であった。
結論
調査した欧州の病院において、CDI 管理のためのガイドラインに対する高い認識が認められた。しかし、一部の国々において、診断検査ガイドライン、CDI 疑い例に対する感染制御策への低い遵守率、ならびに治療ガイドラインへの不十分な認識が、依然として報告された。
