長期ドレーン留置の有害作用と直接吸引の重要性：後向きコホート研究★★

背景
長期の予防的ドレーン留置は、逆行性感染症の原因となる可能性がある。
目的
ドレーン抜去のタイミングと臨床転帰との関連を検討すること。
方法
本後向き単一施設コホート研究では、2016 年から 2020 年に待機的消化管または肝膵胆手術を受け、その後に臓器・体腔手術部位感染症（SSI）を発症した患者 110 例を評価した。予防的ドレーンと直接吸引で培養陽性を認めた菌種の違いについて、予防的ドレーンが SSI 診断時に効果的に機能していたかどうかについて、またドレーン留置前に投与された経験的抗菌薬が検出されたすべての菌種に対して効果的であったかどうかについて評価した。最後に、早期（SSI 診断時にすでに予防的ドレーンが抜去または再留置されていた場合）と晩期（診断後の抜去）のドレーン抜去について臨床転帰を比較した。
結果
予防的ドレーンが SSI 診断時に効果的に機能していたのは、27 例（25％）のみであった。直接吸引の培養結果により、患者の 43％で抗菌薬の escalation が必要となった。ドレーンの抜去または初回再留置までの時間の中央値は、術後 7 日であった。早期抜去群には 43 例（39％）が含まれた。早期抜去と比較して、晩期抜去では結果としてバンコマイシン使用の頻度が高かった（7.0％ 対 22.4％、P ＝ 0.037）。
結論
長期の予防的ドレーン留置は、バンコマイシンを必要とする複雑な感染症と関連する。したがって、ドレーンは可能な限り速やかに抜去すべきである。さらに、抗菌薬療法の escalation は培養結果に基づいて行われることが多いため、直接吸引の培養を行うことを積極的に考慮すべきである。

慶應義塾大学からの単一施設コホート研究。消化器手術の長期ドレーン留置によりバンコマイシンを必要とする感染症が増加するという問題点を浮き彫りにしている。ドレーンの早期抜去と臓器・体腔 SSI 診断時の直接吸引の重要性が理解できる。ドレーン留置期間の長い日本向けの報告で、ぜひ一読をおすすめしたい。
