結腸直腸手術後の手術部位感染症における病院の役割:マルチレベルロジステック回帰分析

背景
手術部位感染症(SSI)は健康転帰不良と関連する。SSI 発生率は、結腸直腸手術後に最も高く、近年ほとんど改善がみられていない。病院の特性が果たす役割は明らかにされていない。
目的
結腸直腸手術後の SSI 発生率に病院間でばらつきがあるかどうか、またそうしたばらつきが病院の特性によって説明されるかどうかを検討すること。
方法
データは、2015 年から 2019 年について Directorate General of Health の電子システムから抽出した。病院の特性は、ポルトガルの行政組織について公表されているデータから抽出した。解析では、2 つのレベルから成る階層データ構造を考慮し、個人を病院でクラスター化した。過適合を避けるため、病院の特性を 2 つ以上含めたモデルは構築しなかった。クラスターレベルの関連は、中央オッズ比(MOR)および級内クラスター係数(ICC)で示す。β相関係数を用いてコンテキスト効果を評価した。
結果
病院 18 施設の計11,219 件の手術を対象とした。SSI 発生率は 16.8%であった。帰無モデルの ICC は 0.09 であった。手術に関する変数によりばらつきの 25%が説明され、病院の規模によりさらに 17%が説明された。SSI のばらつきの 50% 超は説明されなかった。補正後、病院間の不均一性(MOR 1.51、ICC 0.05)が依然として見出された。病院の特性で SSI と有意に関連するものはなかった。
結論
手術に関する変数および病院の規模が SSI のばらつきのほぼ半分を説明しており、予防戦略を導入する場合はこれらを考慮に入れるべきである。今後の研究では、結腸直腸手術の SSI 率が大幅に低い病院における予防バンドルへの遵守状況ならびに手術に関するその他の指標に焦点を当てるべきである。
