抗菌薬の使用を減らすための感染予防と抗菌薬適正使用支援に対する財政的インセンティブ:日本の全国的な観察研究★★

背景
日本政府は病院環境における全国的な抗菌薬の使用を減らすため、財政的なインセンティブを導入した。
目的
本研究は、2012 年に導入された感染予防・制御(IPC)チームの結成に対する全国的な財政的インセンティブ、ならびに 2018 年に導入された抗菌薬適正使用支援(ASP)チームの結成に対する全国的な財政的インセンティブが、全国レベルで抗菌薬の使用と医療資源の利用の変化に関連しているかどうか明らかにすることを目的とした。
方法
2011 年から 2018 年の会計年度中に、日本の全国的な入院患者の代表データベースを用いて、472 の医療施設の感染症入院患者 3,057,517 例を対象とする時系列分析と差分の差分法研究を実施した。主要アウトカムは、100 患者日あたりの抗菌薬治療日数(DOT)とした。副次的アウトカムは、使用された抗菌薬の種類、医療資源の利用および死亡率とした。
結果
2012 年から 2018 年の期間中に計 5,201,304 件の財政的インセンティブが観察され、日本円で計 121 億円(およそ 1.1 億 US ドル)という結果になった。時系列分析によって、2011 年から 2018 年に、全抗菌薬の使用(79.3 ~ 72.5 DOT/100 患者日[8.6% 減少])およびカルバペネムの使用(9.0 ~ 7.0 DOT/100 患者日[7.8% 減少])に減少傾向が確認されたが、その他の医療関連アウトカム(例えば死亡率)に悪影響はなかった。差分の差分分析では、日本の北部を除き、ASP チームに対してインセンティブを与えた病院と与えなかった病院の間で、全抗菌薬の使用に重要な変化は観察されなかった。2011 年 から 2019 年の期間中に財政的インセンティブの金額と抗菌薬の使用の減少との間に、用量反応関係は観察されなかった。
結論
日本の病院環境における抗菌薬適正使用支援を促進するため、さらなる研究と努力が必要である。
監訳者コメント:
日本の感染防止対策チーム(ICT、本論文では IPC チーム)と抗菌薬適正使用支援チーム(AST、本論文では ASP チーム)の活動に関する診療報酬による影響を経時的に抗菌薬使用状況で評価したものである。すなわち、2012 年からの感染防止対策加算 1(400 点)と加算 2(100 点)、および 2018 年からの抗菌薬適正使用支援加算(100 点)がインセンティブとして付いた。一方、2016 年には AMR 薬剤耐性アクションプランが日本政府から出され、抗菌薬使用および耐性に関する目標値が掲げられたが、まだその目標値には至っておらず、財政的インセンティブによる効果は部分的にとどまっている。診療所での抗菌薬使用も決して少なくはなく、医療現場全体での抗菌薬適正使用の方策を考えるべき時期に来ている。
