入院患者の感染予防制御の意思決定における SARS-CoV-2 シークエンシングの所要時間に影響する因子:COG-UK HOCI 試験によるデータの分析★

背景
シークエンシングの結果の迅速な返却に対する阻害因子は、感染予防・制御の決定のための配列データの有用性に影響を及ぼす可能性がある。
目的
COVID-19 Genomics UK Hospital-Onset COVID-19 Infection(COG-UK HOCI)試験において、迅速な所要時間を得るために施設が直面する課題を特定することを目的に、混合法分析を実施した。
方法
定量分析では、シークエンシングプロセスの各段階に関する時点を、COG-UK HOCI 試験のデータセットと研究施設の調査の両方より抽出した。迅速な所要時間の達成に対する阻害因子および促進因子に関する定性データを主題分析により収集した。
結果
感染制御チームによるサンプル採取から配列報告書受理までの全所要時間は、施設間でさまざまであった(中央値 5.1 日、範囲 3.0 日から 29.0 日)。COVID-19 の PCR 検査陽性の結果報告から配列報告書作成までに最大のばらつきが認められた(中央値 4 日、範囲 2.3 日から 27.0 日)。より詳細な分析において、このばらつきの大部分は、COVID-19 の PCR 検査結果からシークエンシング検査機関へのサンプル到着までの遅延の差によることが明らかにされた(中央値 20.8 時間、範囲 16.0 時間から 88.7 時間)。定性分析では、シークエンシング検査機関から診断検査機関までより近接していること、職員の柔軟性が高いこと、および規則的な配送時間が、所用時間の短縮を促すと示唆される。
結論
病原体ゲノムシークエンシングを診断検査施設へ統合することにより、シークエンシングの所要時間の改善を促し、感染制御実践において配列データが実際的価値を有するものになるであろう。精度管理の段階をワークフローの上流に追加し、さらに下流での能力を高めることで、初期段階で低品質のサンプルが除去されるなら、所用時間も最適化する可能性がある。
監訳者コメント:
これまで次世代シーケンサーによる全ゲノム解析は変異株の監視が主であるため、検体採取から結果報告までの時間は問題視されてこなかった。新型コロナの出現によりシーケンサーの進歩により全ゲノム解析が数日以内で結果報告が可能となっており、感染予防対策での新たな利用的価値が生まれている。本論文では、さらにシーケンシングのための検体提出から解析結果報告までの時間の最適化に必要な課題をあげている。
