集中治療室における手指衛生遵守を観察するためのアイトラッキング：無作為化実現可能性研究★

背景
医療関連感染症は患者の死亡率上昇に関連している。手指衛生はこれらの感染症を減少させる最も効果的な方法である。この簡単な介入の簡易化にもかかわらず、手指消毒の遵守率は低いままである。現在の手指衛生評価は主に衛生専門家による観察に基づいている。本研究の目的は、集中治療室における医療従事者の手指衛生遵守の解析中に、アイトラッキングがもたらす付加的な利点を検討することであった。
方法
スイスのチューリッヒ大学病院（University Hospital Zurich）の集学的集中治療室で実施された模擬無作為化クロスオーバー試験で、医師と看護師は、アルコールディスペンサーの位置が異なる（「視野内」と「視野外」）2 つのシナリオでアイトラッキングを受け、2 つの日常業務（中心静脈ラインからのノルエピネフリン 10μg の注入、中心ラインからの採血）を完了した。主要アウトカムは、両方のシナリオについて、3 つの関心領域（中心静脈ライン、アルコールディスペンサー、保護手袋の箱）における滞留時間、再視、最初の注視時間および平均注視時間とした。手指衛生ガイドラインの遵守率を解析した。
結果
参加者 49 名（看護師 35 名、医師 14 名）が本試験の対象となった。アイトラッキングによって、従来の観察と比較してさらなる有用な情報がもたらされた。すべての関心領域について、滞留時間、再視、最初の注視時間および平均注視時間は、2 つのシナリオ間で差は認められなかった。推奨された手指衛生基準の全体の遵守率は、医師（平均 20％）と看護師（平均 42.9％）の両方で低かった。
結論
従来の観察と比較して、アイトラッキングによってさらなる有益な洞察がもたらされ、集中治療室における手指衛生遵守の記録中の注視パターンが徹底解析できた。
監訳者コメント：
アイトラッキングデバイスを用いた 2 つのシナリオでの手指衛生の遵守率の調査。こうした技術は実際の手指衛生の遵守率調査に使用するのは難しいが、教育には利用できそうである。
