背景

多剤耐性病原体(MDRO)保菌の有病率が増加しつつあることは、医療費の増大をもたらすため、医療にとって大きな課題となっている。最近、糞便細菌叢移植(FMT)が、MDRO を除菌するための新規かつ効果的な方法として議論されている。

目的

MDRO 保菌患者における除菌の成功率を最適化するために、異なる FMT の方法について有効性を比較すること。

方法

本前向きコホート研究は、2018 年から 2021 年に MDRO 保菌を認めた患者を登録した。患者には以下のいずれかの方法で FMT が実施された:経口カプセル、上部消化管内視鏡(EGD)、大腸内視鏡、または胃チューブ。

結果

患者計 57 例が、MDR 保菌に対して FMT を受けた。除菌のために要した時間は大腸内視鏡群で最短であった一方、EGD 群では最長であった(24.9 日 対 190.4 日、P = 0.022)。除菌率については、経口カプセル群は EGD 群と同様であった(84.6% 対 85.7%、P = 0.730)。除菌失敗と関連した有意な臨床因子は FMT 後の抗菌薬使用であった(オッズ比 = 6.810、P = 0.008)。全 4 群において、FMT 後のマイクロバイオームの分析から MDRO に属する菌種の割合が減少したことが示された。

結論

他の通常の方法と比べて、経口摂取を忍容できる患者において経口カプセルは効果的な FMT 実施方法である。FMT 後に抗菌薬を中止することは、除菌を成功させるための鍵となる因子である。
