ヒータークーラー装置由来の水サンプルにおけるマイコバクテリウム・キマイラ（Mycobacterium chimaera）の検出率の長期にわたる増加★

背景
マイコバクテリウム・キマイラ（Mycobacterium chimaera）やその他の非結核性抗酸菌（NTM）の定着は、いくつかの製造者により生産されたヒータークーラー装置について報告されている。これまでのところ、LivaNova（ロンドン、英国）のみが心臓手術後の M. chimaera 感染症と関連することが報告されている。ヒータークーラー装置の定着に関する大部分の研究は横断研究であった。
目的
ヒータークーラー装置の水サンプルにおける抗酸菌増殖の長期にわたる動態を調べることを目的とし、LivaNova 3T ヒータークーラー装置 5 台を対象とした前向き抗酸菌サーベイランスのデータを分析した。
方法
LivaNova ヒータークーラー装置 5 台を対象として前向き抗酸菌サーベイランスを行った。各ヒータークーラー装置およびすべてのヒータークーラー装置について、抗酸菌検出の結果を分析した。ロジスティック回帰を適用して、何らかの NTM または M. chimaera の増殖と、ヒータークーラー装置の使用期間との関連についてモデルを作成した。
結果
NTM は、水サンプル計 665 個中 319 個で分離された（48.0％、水サンプル 21 個で、複数の種類の抗酸菌が検出された）。最も高頻度に検出された菌種は、M. chimaera（N ＝ 247/319、77.4％）、マイコバクテリウム・ゴルドナエ（Mycobacterium gordonae）（46/319、14.4％）、Mycobacterium paragordonae（34/319, 10.7％）であった。検出率は、何らかの NTM（使用年あたりオッズ比［OR］：1.60、95％信頼区間［CI］1.17 ～ 2.24、P ＜ 0.001）および M. chimaera（使用年あたり OR：1.67、95％CI 1.11 ～ 2.57、P ＜ 0.01）について、前向きに上昇した。
結論
ヒータークーラー装置の使用期間がより長いことは、何らかの NTM および M. chimaera の検出率上昇とそれぞれ関連していた。
監訳者コメント：
ヒータークーラー装置に関連した心臓手術後の Mycobacterium chimaera アウトブレイクが報告されている。著者らはヒータークーラー装置の非結核性抗酸菌（M. chimaera含む）について前向きサーベイランスを行い、M. chimaera 検出率の経時的な増加を認めた。現在の消毒プロトコールは不十分なため、改善が必要である。
