空気清浄機 Novaerus NV800 について整形外科手術時における空気中バイオバーデンを低減する効果を評価する★

背景
空気清浄機（Novaerus Protect 800）の局所的な設置は、集中治療室内の空気中バイオバーデンおよび医療関連感染症の発生率を低減することが示されている。
目的
このタイプの空気清浄機が、整形外科手術時の手術室内の空気中における細菌濃度を低減し、それによって手術部位感染症のリスクを低減できるかどうかを検討すること。
方法
2018 年に実施した本前向き実験研究では、2018 年にスウェーデンの病院 1 施設の手術室 1 室に 3 台の空気清浄機を設置した。手術 11 件の施行時に、空気清浄機のスイッチをオンまたはオフの状態で、スリット間寒天インパクターにより空気の能動的サンプル採取を行った。モックアップ研究において、煙を利用して空気の動きを視覚化した。
結果
空気中の細菌濃度には、2 つの条件下（空気清浄機のスイッチをオフまたはオン）で有意差がなかった（P ＝ 0.54）。手術創の周囲および上における空気の動きは無秩序で、希釈混合空気の動きと類似していた。
結論
手術室内に設置した 3 台の空気清浄機により、整形外科手術時のクリティカルゾーンにおいて空気中の細菌濃度は減少しなかった。
監訳者コメント：
空気清浄機の局所的な設置は、ICU の空気中バイオバーデンおよび医療関連感染症の発生率を低減するが、整形外科手術時のクリティカルゾーンにおいて空気中バイオバーデンを低減しなかった。これは ICU に比べると手術室での空気の代謝回転が高いためと考えられる。
