Implementation of Public Health England infection prevention and control guidance in maternity units in response to the COVID-19 pandemic

背景

本研究は、新型コロナウイルス感染症（COVID-19）パンデミック中の英国の産科病棟における英国公衆衛生庁（PHE）感染予防・制御ガイダンス実行の成功と障壁について調査することを目的とした。

方法

2020 年 3 月から 2021 年 7 月の間に、英国・イングランドの産科病棟 1 棟に勤務した産科医、助産師、新生児科医との定性的半構造化面接。主題分析を実施した。

結果

PHE ガイダンス実行の成功には、現在のインフラ、訓練の満足度および組織文化が関連しており、組織文化では副主題として集学的アプローチ、COVID-19 に特化した役割および病院全体のコミュニケーションの重要性が検討された。実行の障壁には、更新、専門領域および病院の間の食い違いを強調する副主題を伴うガイダンスの適用性、ガイダンス更新の望ましくないタイミングや頻度、スタッフの遵守の低下および実行の遅れが関連した。最後に、一部の病棟の配置がガイダンスの様々な側面（例えばソーシャル・ディスタンシング）の実行を困難にし、多くの詳細な課題は、情報技術の適合性、適切な個人防護具（PPE）の入手可能性とアクセスの不足、病棟間の検査手配のばらつきに関連した。

結論

本研究は、COVID-19 パンデミック中に産科病棟に勤務していた医療専門家の経験に関する情報を提供するものである。調査結果によって、病院全体の効果的なコミュニケーションの重要性、一貫性があり容易に理解できるガイダンスの必要性が示されている。これらの結果は、専門家パネルによるコンセンサス会議の内容への情報提供に使用される可能性がある。

監訳者コメント：

感染対策などのルールの周知や徹底は、結局感染対策そのものというよりは、ルールの周知や徹底というシステムの問題に立脚している。感染対策はできていないが、医療安全はできている、という病院は存在しない。COVID-19 によって、システム化やガバナンスの問題が浮き彫りになったのではないだろうか。
