Effective cleaning and decontamination of the internal air and water channels, heads and head-gears of multiple contra-angle dental handpieces using an enzymatic detergent and automated washer-disinfection in a dental hospital setting

背景

歯科用ハンドピースは再利用可能で侵襲的な医療機器で、使用後に洗浄、除菌、注油、および蒸気滅菌を行わなければならない。歯科用ハンドピースは複雑な内部デザインを有しており、その中には複数の細いチャネルが含まれ、それらの汚染は消毒の有効性を損い得る。歯科用ハンドピースは、日常的な分解を想定したデザインはなされておらず、洗浄／除菌の有効性のモニタリングが難しい。洗浄消毒は歯科用ハンドピースの除菌には好ましい方法であるが、同法について内部の細菌汚染を低減するための直接的な有効性を調べた研究はほとんどない。

目的

歯科用コントラアングルハンドピースをモデルシステムとして用いて、複数の歯科用ハンドピースにおける内部構成部品の細菌汚染を低減する上での洗浄消毒の有効性を検討すること。

方法

空気および水チャネルならびに分解した 10 台の歯科用コントラアングルハンドピース（BienAir社、Biel/Bienne、Switzerland）のヘッドに対して、0.3％ウシ血清アルブミン（BSA）（清潔条件）、3.0％ BSA、または 10％試験用汚染物質（汚染条件）の存在下で、それぞれ 10⁸ コロニー形成単位（cfu）の緑膿菌（Pseudomonas aeruginosa）、黄色ブドウ球菌（Staphylococcus aureus）、エンテロコッカス・ヒラエ（Enterococcus hirae）、またはカンジダ・アルビカンス（Candida albicans）を播種した。再組立て後、歯科用ハンドピース 10 台すべてに、Míele（Míele Ireland Ltd.、Dublin、Ireland）PG8528 洗浄消毒器により同時に洗浄消毒を行い、微生物および蛋白質の減少について試験した。注油した 3 台の歯科用ハンドピースに黄色ブドウ球菌および 10％試験用汚染物質を播種して、追加の実験を行った。すべての実験を 3 回反復して行った。

結果

平均して、すべての歯科用ハンドピースに対するすべての試験条件下で実施した洗浄消毒後に、歯科用ハンドピースのヘッドとチャネルにおいて、微生物の cfu には約 5 log 以上の減少が、また蛋白質には 93％超の減少が一貫して認められた。

結論

洗浄消毒によって、複数の歯科用ハンドピースの内部構成部品に対する洗浄および除菌を効果的に行うことができる。

歯科病院の設定で、酵素洗剤を使用したウォッシャー消毒により、複数の歯科用ハンドピースを内部および外部から同時に除染することが可能であることを調べた研究。本研究はコントラアングルハンドピースに限定されており、エアタービンハンドピースには適応されない点に注意が必要である。
