背景
医療従事者において密着性の高いマスクは空気感染に対する重要な呼吸器防護具である。とはいえ、顔面にひげのある医療従事者には推奨されない。宗教上または医学上の理由で、ひげを剃ることができない医療従事者もいる。顔面にひげのある医療従事者が密着性の高いマスクを着用できるような解決策として、マスク下での顎ひげカバーが提案されている。しかし、これまでの研究は、定量的フィットテスト法よりむしろ定性的フィットテスト法に主に頼っていたことによる限界がある。
目的
密着性の高い使い捨て P2/N95 マスクの十分な密着を得るために、マスク下での顎ひげカバーの有効性を定量的フィットテストにより評価すること。
方法
顎ひげのある成人男性を対象に、最大 5 種類の使い捨て P2/N95 マスク(3M 1860、3M 1870+、BYD N95 Healthcare Particulate Respirator、BSN Medical ProShield N-95 Medium、Trident RTCFFP2)下に顎ひげカバーを着用した状態で、TSI PortaCount Respirator Fit Tester 8038 または AccuFit 9000 PRO fit 計測装置のいずれかを用いて定量的フィットテストを実施した。主要評価項目は、マスク下での顎カバーを用いた定量的フィットテストに合格または不合格の被験者の割合とした。
結果
30 例を評価し、このうち 24 例(80%)が、少なくとも 1 種類の密着性の高い使い捨て P2/N95 マスクによる定量的フィットテストに合格した。これらの被験者において、達成した最大のフィットファクターの中央値は 200(四分位範囲 178 ~ 200)であった。マスク下での顎ひげカバーに対して有害反応を示した被験者はいなかった。
結論
顔面のひげを剃れない医療従事者において、密着性の高い P2/N95 マスクによる十分な密着を得るために、マスク下での顎ひげカバー法を使用することができる。
監訳者コメント:
顎ひげのある人に対して、ひげカバーを装着したうえで N95 マスクを着用したときのフィット状況を定量的に計測した論文である。結果を見ると、N95 の種類によっては、30 例中 19 例が不合格となっているものもあり、改めて、顎ひげのある人を含め、フィットテストの重要性を感じた論文であった。
