Assessing changes in stethoscope hygiene during COVID-19: a multi-centre cross-sectional study

背景

新型コロナウイルス感染症（COVID-19）（重症急性呼吸器症候群コロナウイルス-2［SARS-CoV-2］）パンデミックにおいて、患者へのさまざまな接触法によって感染制御のための警戒が強化されてきた。しかし、それに伴って、病原菌汚染の可能性がある聴診器に関する衛生管理も変化しているかどうかは不明である。

目的

パンデミックによる聴診器の衛生管理の変化を明らかにすること。

方法

主要な医療センター 3 施設において医療従事者を調査した。COVID-19 発生前と期間中の 2 つの構成要素からなる質問票（Likert 尺度と頻度）で、聴診器の衛生管理に関する考えと実践について定量的に評価した。COVID-19 発生前と期間中における聴診器の最適な衛生管理の実施（各患者への使用後）に基づき参加者をグループ分けした。χ² 検定、分散分析（ANOVA）によりグループ間で比較した。

結果

調査を終了した 515 名（10％）のうち 55 名を除外した（460 名）。最適な衛生管理は 27.4％から 55.0％に増加した（P ＜ 0.001）。聴診器汚染の知識に関するすべての質問の Likert スコアは有意に増加した（P ＜ 0.001）。衛生教育の理解に変化はないものの、聴診器汚染に対する考えは高まった（P ＜ 0.001）。研修医は、指導医や看護師と比較して、COVID-19 期間中の最適な衛生管理を実施した割合が低かった（P ＜ 0.001）。

結論

COVID-19 期間中に聴診器の衛生管理が改善されたにも関わらず、最適な衛生管理が実施されたのはまだ医療従事者の約半数にとどまった。特にキャリアが浅い医療従事者を対象とした教育的介入を奨励する。
