背景
医療従事者、患者、病院環境へのメチシリン感受性黄色ブドウ球菌(meticillin-susceptible Staphylococcus aureus[MSSA])の定着が、MSSA 伝播イベントに及ぼす作用は十分に理解されていない。
目的
MRSA の 2 年間におよぶ流行歴がある大規模病院において、非アウトブレイク時のメチシリン耐性黄色ブドウ球菌(meticillin-resistant Staphylococcus aureus[MRSA])の作用を、全ゲノムシークエンシング(WGS)により近年検討した。多数の潜在的な MRSA 伝播イベントが特定された。本研究では、同 2 年間の病院研究期間中の同じ感染源による MSSA 伝播イベントについて検討した。
方法
9 病棟の医療従事者(326 名)および患者(388 例)のボランティアを対象に、鼻腔、口腔 MSSA保菌について 2 年間にわたり検査した。患者が近接する環境(1,164 個)、接触頻度が高い場所(810 個)、空中(445 個)のサンプルのMSSA スクリーニングを実施した。MSSA 代表株および臨床分離株を配列決定し、コアゲノム複数部位塩基配列タイピングにより分析した。近縁株(対立遺伝子の差異 24 以下)を類縁株群(RIG)に分類した。医療従事者、患者、患者感染症からの RIG に MSSA を含む潜在的伝播イベントを、疫学的データと組み合わせて特定した。
結果
全体で MSSA 635 株を回収し、内訳は臨床分離株(82 株)、医療従事者(170 株)、患者(120 株)、環境分離株(263 株)であった。配列決定された MSSA 635 株のうち 406 株で24 種類の clonal complex(CC)が同定され、この 406 株のうち 183 株が 59 種の RIG に分類された。医療従事者から患者、医療従事者間、患者間で多数の潜在的伝播イベントが、主に CC5-MSSA、CC30-MSSA、CC45-MSSA において確認された。医療従事者、患者、臨床、環境からの分離株は、それぞれ 33 種の RIG、24 種の RIG、6 種の RIG、32 種の RIG で同定され、この 32 種のうち 19 種は医療従事者および/または患者の分離株の類縁 MSSA 株であった。
結論
非アウトブレイク時の医療従事者、患者、環境の汚染に影響を及ぼす多数の潜在的な MSSA 院内伝播が、WGS により認められた。

非アウトブレイク時のメチシリン感受性黄色ブドウ球菌の院内伝播状況を全ゲノムシークエンスと疫学解析で明らかにした論文である。非アウトブレイク時の伝播を「見える化」することは、その先のアウトブレイクの発生を阻止することができる貴重な研究である。
