Patients’ perspectives of healthcare-associated infection: ‘you don’t know what impacts it will have on your life’

背景

医療関連感染症（HAI）は、入院時には存在しないが、入院患者が病院で治療を受けている間に罹患する。HAI の有病率、およびその原因と治療については、これまでに様々な見地で研究されてきた一方、HAI に罹患すること、またその後遺症とともに生きることに関する患者の視点については、いまだ十分に研究されていない。

目的

HAI に罹患するという患者の経験を調査すること。

方法

半構造化面接から得られたデータに基づく、定性的な現象学的研究を実施して、最近病院で HAI に罹患した患者の得られた経験の調査を試みた。参加者は、2019 年と 2021 年にオーストラリアの病院 2 施設で募集した。参加者 10 例に対して、研究チームのメンバー 2 名が電話面接を実施し、これらの面接の筆記記録をテーマ別に分類するプロセスを用いて定性的に分析し、HAI に罹患することに関する患者の視点を明らかにしようとした。

結果

参加者は、様々な HAI に罹患していた。HAI に罹患するという経験は、患者の視点からいかに非常に孤立させられ、またつらいものであり得るとともに、生涯にわたる影響があるかについて参加者は述べた。このことは、HAI に罹患した結果として、患者が感情的および精神的にどのような影響を受けるかを理解するために役立つ。

結論

HAI の感染源、治療および予後について、患者中心のコミュニケーションを改善する必要がある。本研究の結果から、患者の医療において、患者自身の声を考慮することの重要性が示されている。
