医療従事者の靴底から採取されたクロストリジオイデス・ディフィシル(Clostridioides difficile)株は感染患者と疫学的関連があり、全ゲノムシークエンシングにより一致が確認された

背景
医療施設におけるクロストリジオイデス・ディフィシル(Clostridioides difficile)の感染源は不明であることが多い。患者由来と医療従事者の靴底由来の培養株の関連は、手術室では既に認められているが、急性期施設の一般病棟では明らかにされていないので、本前向きコホート研究では、この関連を評価することを目的とした。
方法
スイスの大学附属 3 次医療施設において研究を実施した。2019 年 10 月から 2020 年 7 月まで、毒素産生 C. difficile 感染患者のケアにあたる医療従事者のシフトごとに 2 回、靴底の C. difficile を培養した。関与した医療従事者との面接により他のリスク因子を評価した。患者の糞便サンプルをルーチンの微生物学的方法で処理した。医療従事者と患者の菌株の類似性を全ゲノムシークエンシング(WGS)により明らかにした。
結果
入院患者 42 例に接触した医療従事者計 103 名が研究に参加し、206 サンプルが採取された。C. difficile 感染患者をケアした医療従事者において、靴底の C. difficile 汚染が 37 サンプル(17.8%)で検出された。全体的にみて、疫学的関連と WGS により確認された菌株の一致によって 74%で伝播が疑われた。
結論
医療従事者の靴底の C. difficile 陽性率は 17.8%であった。この菌株は感染患者と疫学的に関連しており、WGS により確認されたことで、医療従事者の靴底による潜在的伝播が示唆される。本パイロット研究は、C. difficile の医療関連感染におけるこの潜在的伝播について、より大規模な臨床試験でさらに評価するための十分なエビデンスを提供するものである。
監訳者コメント:
床から回収した C. difficile が陽性ということは一つの環境汚染の指標とはなるが、靴底を介して伝播しているという証拠とは言い難い。
