感染制御策バンドルは、黄色ブドウ球菌（Staphylococcus aureus）による術後胸骨創感染症を低減させるが、グラム陰性菌に起因する場合は判別できない：患者エピソード 6,903 件の後向き分析

背景
心臓手術部位感染症の予防では、主として黄色ブドウ球菌（Staphylococcus aureus）による感染症低減に重点が置かれているが、他の細菌もこの合併症に重要な影響を及ぼす。
目的
進展している感染制御プログラムが胸骨創感染症の発生率に及ぼす影響およびブドウ球菌以外による感染症の発生率の変化を評価すること。
方法
英国の単一施設において 2010 年 9 月から 2018 年 5 月に初回胸骨切開術を受けた全患者の後向きコホート研究を実施した。S. aureus 除菌を含む広範な感染制御プログラムの段階的導入に先立つ 2 年間のデータを照合した。
結果
計 6,903 件の初回胸骨切開術を実施し、そのうち 2.6％（178 件）が胸骨創感染症を併発した。原因菌として高頻度に同定されたのはグラム陰性菌（GNB）と S. aureus であった（それぞれ 45.5％、30.3％）。胸骨創感染症の発生率は 3.9 件／100 例／月から、プログラム導入後には 1.8 件／100 例／月に減少した。これは主に、S. aureus 感染症症例の持続的な減少によるもので、グラム陰性菌への影響は判別できなかった。多変量ロジスティック回帰分析により、術後感染症の独立リスク因子として冠動脈バイパス術、手技上の緊急性、暦年第 3 四半期（7 月から 9 月）に実施された手術が特定された。
結論
多面的な感染制御プログラムは、主に S. aureus 感染症の低減によって胸骨創感染症発生率の低下に奏効した。GNB も胸骨創感染症に重要な影響を及ぼしており、従来の予防策ではこれに対処できない。今後の介入および影響の評価において、有効性を評価する場合、GNB 感染症を考慮すべきである。
