医療従事者の指先の細菌の不活化に対する銀ポリマー、乳酸、ベンザルコニウム塩化物を利用した 3 種類の手指消毒用非アルコール製剤の有効性の低さ★

背景
最近、新たな手指消毒用非アルコール製剤が発売された。それらは欧州規格 EN1500 試験に合格しているが、擦式アルコール製剤と比較した臨床効果に関するデータは不十分であり、主にベンザルコニウム塩化物(BAC)を対象としている。
目的
泡状の手指消毒用銀ポリマー製剤、乳酸製剤、BAC 製剤およびゲル状擦式アルコール製剤が、病棟で勤務している医療従事者の指先の細菌数を減少させる効果について試験すること。
方法
84 名の参加者はそれぞれ、製品で手指を擦る前と擦った後に「ペトリ皿に指先を置く」手法を用いて、病棟の午前の勤務中に、4 種類の製品のうち 1 種類を試験した。培養後、2 名の独立した読み取り担当者が培養プレート上の細菌数を評価した。
結果
本試験の条件では、擦式アルコール製剤は試験者の指先の細菌を効率的に減少させた一方で、乳酸および BAC による消毒薬は検出可能な有効性を示さなかった。銀ポリマー製剤はある程度の効果を示したが、さらなる調査が必要である。
結論
擦式非アルコール製剤は、臨床での用途が認められる前に、注意深い検討とさらなる研究を必要とする。
監訳者コメント:
新型コロナウイルス感染症のパンデミックにより感染対策への有効性を謳う様々な製品(消毒剤、抗菌製品、マスクなどの防護具、空気清浄機など)が世の中に氾濫している。フィンランドにおける手指消毒剤の効果を再検証している。EN 1500 は、人為的に細菌汚染させた後に手指消毒剤を使用し、指先に残っている生菌数を測定することで、衛生的な手指消毒剤の有効性を評価する欧州規格の試験方法であるが、一応実際の場面を想定して作成された基準ではあるが、本論文で実施された追加調査(フィンガーチップ法)では模擬細菌(一過性菌)は使用せず、常在細菌を前後で定量し、その有効性が認められなかった。
