ネパールの 3 次病院における mcr-10を保有するEnterobacter kobeiによる新生児敗血症の致死的アウトブレイク★★

背景

Enterobacter kobei は、新生児集中治療室(NICU)における院内感染症アウトブレイクの新たな原因となっている。2016 年 7 月から 9 月にかけて、ネパールの 3 次病院の NICU において、エンテロバクター(Enterobacter)属菌を原因とする新生児敗血症の発生数に急激な増加がみられ、入室していた新生児 23 例中 11 例が感染し、うち 5 例が敗血症の悪化により死亡した。

 

目的

疑わしいアウトブレイクを確認すること、環境内の感染源を同定すること、およびこの病原体における抗菌薬耐性および病原性の遺伝的決定因子の特徴を明らかにすること。

 

方法

NICU に入室している敗血症の新生児を対象に、2016 年 5 月から 2017 年 12 月に血液培養で分離されたすべてのエンテロバクター属菌に対して、全ゲノムシークエンシングを実施した。またアウトブレイクの期間中、環境サンプリングを、2 週間に 1 回から週 1 回に強化した。

 

結果

ゲノム解析により、2016 年 7 月から 9 月に新生児の血液培養で分離された重複のない E. kobei 分離株 11 株中 10 株が同一クローンであることが明らかになり、これによりアウトブレイクであることが確認された。分離株は抗菌薬耐性遺伝子を保有しており、これにはそれぞれカルバペネムとコリスチンに対する感受性を低下させる blaAmpC と mcr-10 が含まれた。しかし、環境サンプリングではエンテロバクター属菌はいっさい分離されなかった。侵襲的手技における無菌操作プロトコール、手指衛生、環境除菌、燻蒸消毒、ならびに培養陽性症例の隔離環境でのケアを強化したところ、アウトブレイクを終結させることができた。

 

結論

本研究から、感染症アウトブレイクを防ぐためには、厳格な感染制御策の実施が必要であることが示された。本研究では、NICU における院内新生児敗血症の新たな原因として、遺伝子 mcr-10 を保有し、カルバペネムとコリスチンに対する感受性を有さない E. kobei を初めて報告している。

監訳者コメント:

 Enterobacter kobei 菌血症によるアウトブレイクで、本菌は生化学的性状では E.cloacae と区別できないため E. cloacae complex に臨床的には分類されているが、全ゲノム解析や質量分析の進歩により同定が可能となってきている。染色体性に ampC をもちポーリン減少や耐性プラスミドの追加によりカルバペネム耐性となる。本論文では、キノロン系およびアミノグリコシド系に感受性である。NICU 内の免疫能低下の新生児における侵襲的処置が菌血症の原因となり、手指衛生をはじめとする基本的な感染対策に加え、無菌的処置の徹底、保育器の徹底した清掃消毒は必須である。
