内科集中治療における抗菌薬の費用★

背景
集中治療室は抗菌薬の使用量が多いため、抗菌薬使用量をモニタリングし医薬品の経済性評価研究を実施する主要な対象である。
目的
抗菌薬の使用量および費用を、包括的に、系統ごとに、院内感染症および多剤耐性感染症に関して、評価すること。
方法
本研究は、1 年（2018 年 1 月 1 日から 12 月 31 日まで）にわたる単施設後向き観察研究であり、モロッコ・カサブランカの Ibn Rochd 大学病院の内科集中治療室に入院している全患者を対象としている。
結果
抗菌薬の包括的な年間使用量は、1 日規定用量（DDD）で1,000 床・日あたり 1,410.21 DDDを占め、そのうちβ-ラクタムが最も消費された（1,000 床・日あたり 768.95 DDD）。市中獲得型感染症の年間抗菌薬使用量は 1,340.82 g DDDという結果になったのに対し、院内感染症では 2,483.69 g DDD であった。多剤耐性感染症の年間使用量は 737.5 g DDD（52.2％）という結果になった。処方された抗菌薬の包括的な費用は、118,224.32 US ドルに達した。β-ラクタム（38％）およびフルオロキノロン（21％）の合計使用量には、総予算の約 60％の費用がかかった。院内感染症治療のための抗菌薬に使われた費用は 57,544.11 US ドルであったのに対し、市中獲得型感染症治療では 60,859.41 US ドルであった。多剤耐性感染症治療に処方された抗菌薬の費用は、47,744.14 US ドルに達した。
結論
本研究によって、完全に回避できる一定費用の推定が可能となる誤用と過剰使用の複数の側面を明らかにできるようになった。結果として、この評価は抗菌薬使用の合理化への第一歩となるだろう。
監訳者コメント：
これまでに抗菌薬適正使用に関する活動が十分に実施されていない国、モロッコでの研究であり、全体の使用量と系統別の使用量を算出し、院内感染と市中感染と耐性菌感染での抗菌薬使用状況が把握できたという報告である。一方で、2015 年に WHO が出した耐性菌グローバルアクションプランとともに GLASS（Global Antimicrobial Resistance Surveillance System）という耐性菌サーベイランスが立ち上がり、日本も参加している。モロッコは 2018 年末にこの GLASS に参加しており、ようやく耐性菌の状況が比較可能となってきており、今後抗菌薬適正使用の気運が高まるのではないかと思われ、この論文のデータが活用される日が近いうちにくることを期待する。
