病院および長期ケア施設の環境サービス従事者の清掃と消毒に関するトレーニング、技術、活動、有効性:システマティックレビュー★★

背景
医療施設において、多剤耐性病原体(MDRO)(メチシリン耐性黄色ブドウ球菌[Staphylococcus aureus]、バンコマイシン耐性腸球菌[enterococcus]、多剤耐性アシネトバクター[Acinetobacter]属菌、基質特異性拡張型β–ラクタマーゼ産生グラム陰性菌)、クロストリジオイデス・ディフィシル(Clostridioides difficile)、ウイルス性呼吸器病原体、医療関連感染症(HAI)は大きな懸念となっている。
目的
患者および表面における MDRO や HAI の低減を図るために環境サービス従事者のトレーニング、業務量、各シフトで清掃される表面数・医療器具数、各表面における作業所要時間、清掃法、監視、MDRO と HAI の状況、および有効性指標を評価すること。
方法
CINAHL、Cochrane CENTRAL Register of Controlled Trials、EMBASE、Medline、Scopus を用いて、発端から 2021 年 6 月 28 日まで検索した(言語/日付の制限設けず)。次の 3 つのテーマの用語を組み合わせて検索した。(a)病院および急性期/長期ケア施設(例;介護付き住宅、長期ケア施設、介護施設)(b)消毒薬(例;生体消毒薬、漂白剤、洗浄、銅めっき表面・銅含浸布、非生体消毒薬、汚染除去、過酸化水素、4 級アンモニウム消毒剤、紫外線)(c)無作為化比較試験(RCT)。
結果と結論
クラスター RCT 14 報により、一連の介入に基づくデータが得られた。10 報では、MDRO/HAIによる患者感染数の減少に焦点が置かれ、4 報では、漂白剤、4 級アンモニウム消毒剤、紫外線、蒸気化過酸化水素などの対策により患者感染率の有意な低減が確認された。環境サービス従事者数、MDRO/HAI の状況、業務量、各シフトで清掃された表面数と対象物数、有効性の向上に及ぼすトレーニングの影響に関して得られた情報はわずかであった。結果より、環境サービス従事者の個別およびチームとしてのトレーニングおよび消毒活動の影響に関して詳細な評価を行う必要性が示唆される。
監訳者コメント:
医療環境面の整備を系統立って行い、物理的な清掃に加え新技術の紫外線や蒸気化過酸化水素等と組み合わせることで入院患者の感染リスクが軽減できることが複数の質の高い論文で証明されている。我が国でもこうした取り組みを開始している医療施設が増えてきている。
