ヒンジ式手術器具：バイオフィルム除去に対する二重用手洗浄と自動洗浄の有効性の比較★

ヒンジ式手術器具上のバイオフィルム除去に対し、酵素洗剤後にアルカリ洗剤を用いる二重用手洗浄の有効性を、自動洗浄と比較した。バイオフィルムで覆われた止血鉗子を 4 つのグループに分けた：陽性対照（滅菌水に浸す）、二重用手洗浄、二重用手洗浄とヒンジ内部の追加のブラッシング、自動洗浄。二重用手洗浄、二重用手洗浄とヒンジのブラッシング、ならびに自動洗浄はすべて、陽性対照と比較して、それぞれ生菌の 94.8％、99.8％、100％、残存蛋白の 82.3％、93.8％、95.1％を、有意に減少させた（P ＜ 0.001）。二重用手洗浄を行った器具は、ヒンジのブラッシングを伴う二重用手洗浄および自動洗浄を行った器具よりも、有意に多くの生菌（P ＜ 0.05）および残存蛋白（P ＜ 0.01）を有していた。しかし、ヒンジのブラッシングを伴う二重用手洗浄と自動洗浄との間に、残存蛋白の有意な差は認められなかった。自動洗浄設備を利用できない滅菌サービス部門では、用手洗浄時にヒンジ内部をブラッシングすることが重要であり、二重用手洗浄とヒンジ内部のブラッシングは、ヒンジ式手術器具の洗浄における実行可能な代替案の 1 つと考えられるだろう。
監訳者コメント：
本研究でも結論付けているようにヒンジ式手術器具の洗浄は自動洗浄が最適な選択であるが、自動洗浄設備が利用できない場合は、二重用手洗浄とヒンジ内部のブラッシングは代替案となりそうである。
