重症熱性血小板減少症候群ブニヤウイルスのヒト‐ヒト間伝播の素因★★

重症熱性血小板減少症候群（SFTS）ウイルスのヒト‐ヒト間伝播は、人の健康にとって新たな脅威である。SFTSの院内ヒト‐ヒト間伝播のアウトブレイクについてここに報告する。発端患者と対面での距離が 50 cm 以内および／または曝露時間が 30 分以上であった患者 8 例のうち 6 例が感染した。曝露距離 50 cm 以上および曝露時間 30 分以内であった 17 例のうち感染したのは 1 例のみであった（75％対 6.25％、P ＜ 0.001）。疫学的研究では、ヒト‐ヒト間伝播の重要な因子として高ウイルス量、血性分泌物と出血、曝露時間、距離が確認された。
監訳者コメント：
SFTS は主には SFTS ウイルスを保有するマダニに刺咬されることにより感染するが、本報告にあるようにヒト‐ヒト間伝播が起こりうる。感染経路は明らかではないが、考えられる経路としては血液や血性呼吸器分泌への直接接触だけでなく、飛沫やエアロゾルの関与も疑われる。SFTS が疑われる患者に対応する医療従事者や介護者は N95 マスクやフェイスシールドなどの眼の防護を考慮すべきである。
