日本の黄色ブドウ球菌（Staphylococcus aureus）の多剤耐性表現型のサーベイランスと全ゲノムシークエンスの結果との相関性★

背景
黄色ブドウ球菌（Staphylococcus aureus）の抗菌薬耐性は、大きな疾病負荷をかける。表現型および遺伝子型の両方のモニタリングは、新興耐性菌株の理解と封じ込めの鍵である。
目的
黄色ブドウ球菌の新たに出現した耐性の表現型のモニタリングと、重要度の高い菌株の表現型と全ゲノムシークエンシング（WGS）の結果との相関性。
方法
2011 年から 2019 年までの、213 の参加病院の 40,000 超の分離株の抗菌薬感受性試験の結果を、院内感染対策サーベイランス（JANIS）の全国的なデータベースからエクスポートした。黄色ブドウ球菌の特徴的な多剤耐性の表現型（耐性プロファイル）の長期的・地理的な分布と有病率は、病院および県の間で検討した。さらに、懸念すべき特定の耐性プロファイルを有する菌株のゲノムシークエンス解析を実施した。
結果
メチシリン耐性黄色ブドウ球菌（MRSA）の全体の有病率は、2011 年の 40.3％から 2019 年には 35.1％に低下した。しかし、9 年間で、黄色ブドウ球菌の多数の耐性プロファイルのうち、MRSA の 型の1 つのプロファイルのみが、入院患者の頻度において 10％を超える統計学的に有意な増加を示した。この MRSA のプロファイルは、オキサシリン、エリスロマイシンおよびレボフロキサシンに耐性を示した。この表現型を有する黄色ブドウ球菌分離株の WGS の結果の解析によって、ほとんどは clonal complex 8 に属しており、すべてが市中獲得型 MRSA に特有の SCCmec IV を保有することが明らかになった。
結論
特徴的な耐性プロファイルの追跡によって、MRSA の全体的な減少への理解が深まり、新たな耐性表現型の出現を認識することにつながった。本研究では、多剤耐性表現型と相関する抗菌薬耐性に関する選択的なゲノムシークエンシングを用いた今後の疫学的研究モデルを提供しており、これは他の細菌種に適用できる可能性がある。

日本の MRSA の分離頻度の推移と分子疫学に関する研究。MRSA の分離率は低下したが、いわゆる CA-MRSA は増加した。経時的な増減だけでなく、薬剤感受性の推移や、MLST にも触れている。日本の MRSA の疫学について深掘りしたい人には必須の論文。
