入院中の COVID-19 疑い患者の迅速スクリーニングのための FebriDx：文献のシステマティックレビューとメタアナリシス

医療施設に入院中の新型コロナウイルス感染症（COVID-19）疑い患者のトリアージのための FebriDx 側方流動免疫測定法に関して、文献のシステマティックレビューとメタアナリシスを実施した。「FebriDx」AND「COVID-19」OR「SARS-CoV-2」のキーワードを用いて、言語と日付（2022 年 2 月 4 日に至るまで）いずれの制限も設けず、Scopus、Medlineで電子検索を実施した。選択する研究は、急性期医療施設における COVID-19 疑い患者のトリアージに FebriDx が使用され、2 × 2 分割表を構築できる十分なデータを報告したものであった。5 報が最終解析の対象となり、患者総数は 2,309 例であった。診断の統合感度 0.91（95％信頼区間［CI］0.88 ～ 0.93）、統合特異度 0.92［95％CI 0.90 ～ 0.93］）で、また曲線下面積 0.971（95％CI 0.962 ～ 0.980）、正確度 91.4％（95％CI 90.2％ ～ 92.5％）、kappa 統計量 0.762（95％CI 0.731 ～ 0.793）であり、参照とした分子検査法とかなりの一致度を示した。陰性適中率は 0.974（95％CI 0.966 ～ 0.981）、陽性適中率は 0.742（95％CI 0.711 ～ 0.770）であった。この統合解析により、急性期医療施設（とくに、ウイルス循環が高く、検査前の確率が高い区域）における COVID-19 疑い患者の迅速スクリーニングのために、FebriDx は臨床的有用性があり、確証的な臨床検査の優先順位付けができることが明確にされた。
監訳者コメント：
FebriDx という迅速検査キットを医療施設に入院中の COVID-19 疑い患者のトリアージに利用できるかを文献のレビューで検討した論文である。トリアージという短時間の判断が求められる場面で選択する検査法は極めて重要と思われた。
