A systematic review and meta-analysis of disease burden of healthcare-associated infections in China: an economic burden perspective from general hospitals

医療関連感染症（HAI）は、世界的な公衆衛生上の問題である。しかしながら、中国では HAI に起因する国内レベルでの経済的負担について知られていない。今回のシステマティックレビューの目的は、中国における HAI による直接的な経済的負担を推定することである。2009 年から 2019 年に発表された研究を対象に、Medline、EMBASE、Chinese Journals Online のデータベースを検索した。統合推定値とその 95％信頼区間を分位点の推定により算出した。DerSimonian‐Laird 法のランダム効果モデルを用いた。P ＜ 0.05を統計的に有意とした。計 2,756 報の研究を特定し、メタアナリシスでは、直接的な経済的負担の統合推定値の算出に 6 報、追加の経済的負担の統合推定値の算出に 5 報を対象とした。HAI 患者の 1 入院あたりの総医療費、薬剤費、入院日数の統合推定値の中央値は、それぞれ 34,415.62 日本円、20,065.21 日本円、34.01 日であった（P ＜ 0.0001）。HAI 患者と非 HAI 患者間で、1 入院あたりの総医療費、薬剤費、入院日数の差の統合推定値の中央値は、それぞれ 24,881.37 日本円、9,438.46 日本円、13.89 日であった（P ＜ 0.01）。結論として、HAI 患者の治療費は非 HAI 患者よりも有意に高額であった。この過剰な経済的負担は、患者とその家族のほかに医療サービス提供者、医療制度など全体的に影響を及ぼす可能性がある。HAI を制御するために、有効なサーベイランスシステムと費用対効果の高い介入が必要である。
