免疫不全の小児患者におけるロタウイルスワクチン株の院内感染★

免疫不全の乳児において、ロタウイルス(RV)ワクチンは便中に持続的に排泄される。本稿では、免疫不全小児患者におけるロタウイルスワクチン株の院内感染について分析した。RV1 接種を受けた症例 A と RV1 接種を受けなかった症例 B の便および血清サンプル中に RV1 RNA が検出された。症例 A から連続採取した便サンプルのポリアクリルアミドゲル電気泳動（PAGE）分析により、ロタウイルスゲノムの複数の再配列が認められた。症例 B において検出された特異的なバンドパターンは、同時に症例 A で検出された再配列と同じであった。要約すると、免疫不全の乳児へのロタウイルスワクチン接種は、院内感染のリスクをもたらす。
監訳者コメント：
ロタウイルスワクチンは経口生ワクチンである。免疫不全者へのワクチンの有効性、安全性は確立しておらず、接種要注意者となっている。接種が開始される生後 6 週時点では免疫不全の診断は困難で、免疫不全者にロタウイルスワクチンが投与されると便中に長期にわたり排出される。今回の院内感染は小児の介護者を経由した伝播の可能性が考えられ、特定の状況下では医療従事者だけでなく介護者の接触予防策の実施が必要であることを示唆している。
