Delivery of antimicrobial stewardship competencies in UK pre-registration nurse education programmes: a national cross-sectional survey

背景

登録看護師は抗菌薬適正使用支援の成功に極めて重要な多数の役割を果たしているが、適正使用支援に関する教育課程を含む看護師登録前プログラムは 63％のみである。最新の看護基準では、看護師は抗菌薬適正使用支援の知識と技能を必要とすることが指摘されている。

目的

最新の看護師登録前プログラムにおける重要な抗菌薬適正使用支援能力の提供について調査すること。

方法

本研究は横断的調査デザインで行った。データは 2021 年 3 月から 6 月の間に収集した。

結果

抗菌薬適正使用支援の教育を担当する 35 の英国の大学の専任講師が本研究に参加した。抗菌薬適正使用支援の教育および学習の提供は、プログラム全体で、抗菌薬の使用、管理とモニタリングに関する能力よりも前例を優先する感染予防制御、患者中心のケア、専門職連携による実践の能力と一貫性はなかった。手指衛生、個人防護具および免疫理論周辺のオンライン学習および教育は、パンデミック中に増加したと報告された。生徒が教わった知識をその他の医療専門家のグループと共有したと報告した回答者はごく少数のみであった。

結論

プログラム全体で抗菌薬適正使用支援の一貫性を確保する必要があり、抗菌薬の使用、管理およびモニタリングに関するより深い知識が含まれるべきである。プログラムには、看護師が協力して実践するために専門職連携技能を成長させられるような教育戦略および方法を採用する必要がある。

抗菌薬適正使用支援に最も重要な立ち位置にいるのが病棟看護師であるとの見解が米国看護師協会から数年前に出されている。患者に寄り添い、患者の病態をもっともきめ細やかに経時的に観察している看護師の支援が抗菌薬適正使用支援が欠かせない。電子カルテをツールとして看護記録の機能を生かすことで、こうした観察項目の多くがエントリ可能なはずだ。
