2009 年から 2018 年のイングランドにおける血流感染症を引き起こすシュードモナス（Pseudomonas）属菌分離株の発生率、致死率および抗菌薬耐性の傾向：自発的な全国サーベイランスの枠組みの結果

背景
本稿では、イングランドにおけるシュードモナス（Pseudomonas）属菌による血流感染症（BSI）について、2017 年 4 月から開始された義務的サーベイランスによる将来の所見と比較するため、ベースラインの疫学データを提供する。
目的
2009 年から 2018 年のイングランドにおける Pseudomonas 属菌による BSI の発生率、30 日全死因死亡率および抗菌薬耐性の傾向について報告すること。
方法
患者および抗菌薬感受性のデータを、UK Health Security Agency の自発的サーベイランスデータベースから得た。死亡に関する情報は、中央データリポジトリとのリンクから得た。
結果
2009 年から 2018 年に、Pseudomonas 属菌による BSI が 39,322 件認められた。回帰分析から、発生率は春季（3 月から 5 月）と比べて、夏季（6 月から 8 月）には18.5％高く（P ＜ 0.01）、秋季（9 月から 11 月）には 16.2％高かった（P ＜ 0.01）。30 日全死因致死率は、2009 年の 32.0％から 2018 年には 23.8％に減少した（P ＜ 0.001）。2018 年について、耐性が認められた主要な抗菌薬はシプロフロキサシン（7.5％）、セフタジジム（6.8％）、ピペラシリン・タゾバクタム（6.6％）、カルバペネム系薬（5.5％）、およびゲンタマイシン（4.1％）であった。人口 100,000 人当たりの死亡率は、秋季には 25.7％高く（P ＜ 0.01）、冬季（12 月から 2 月）には23.6％（P ＜ 0.01）高かった。
結論
近年、全般的に症例数の増加がみられるが、Pseudomonas 属菌による BSI 後に死亡する患者（全死因による）の割合は減少している。しかし、他の顕著な医療関連 BSI と比べると全死因死亡率は高い。このことは、対象を絞った感染予防制御戦略を支援するための継続的なサーベイランスの必要性を浮き彫りにしており、患者におけるリスクグループについてさらなる理解と、おそらくは抗菌薬使用実践に有用な情報を提供するものである。
監訳者コメント：
菌血症の原因菌の中でも緑膿菌は比較的稀で、またしばしば侵入門戸の評価が難しい。本研究は検査室サーベイランスが基礎となっているため患者情報が十分に得られていないが、季節性や感受性などを明らかにしている。自施設での菌血症サーベイランスの評価の際に、参考にしてみると面白いかもしれない。
