Effectiveness of air purifiers in intensive care units: an intervention study

背景

集中治療室（ICU）の換気システムの効果的な設計および運用は、病院感染を予防するために重要である。空気清浄機が寄与する可能性がある。

目的

ICU の空気中および高頻度接触面に存在する微生物数および種類を検出すること、ならびに空気清浄機が微生物数を減少させ、結果として ICU における院内感染率を低下させる効果を評価すること。

方法

本介入研究は、2020 年 5 月から 11 月の間に、2 つの類似した ICU において実施された。Novaerus 社製の空気清浄機を、介入 ICU に 2 か月間設置した。対照 ICU では介入 ICU と同様に、ルーチンの清掃手順および HEPA フィルタによるろ過を継続した。結果のあらゆる可能性のあるバイアスを減らすため、2 か月後、空気清浄機をもう一方の ICU に次の 2 か月間移動させた。空気サンプルおよび表面サンプルを評価した。

結果

介入 ICU の経時的な変化の評価によって、60 日目の空気中および表面上のコロニー濃度は、1 日目と比較して、有意に低いことが明らかになった（P air ＜ 0.001、P surface ＜ 0.001）。介入 ICU（r ＝ 0.406、P ＝ 0.049）と対照 ICU（r ＝ 0.698、P ＝ 0.001）において、検出されたコロニー数と病院感染率との間には有意な正の相関が認められた。

結論

病院の空調システムに加えて空気清浄機を使用することは、空気中および表面上の微生物数を減少させ、結果として病院感染を減少させる効果的な方法である可能性がある。

空気清浄度を高く維持することで、患者の気道感染症等が軽減されるのかどうかという、アウトカム評価がなされていない。
