Factors associated with antibiotic administration delay among preterm infants with late-onset bloodstream infection

抗菌薬の早期投与は、早産児の遅発性血流感染症による死亡を減らす重要な変更可能因子の 1 つである。2 つの 3 次新生児集中治療室（NICU）の、コアグラーゼ陰性ブドウ球菌（CoNS）以外の血流感染症を有する 142 例の乳児を含むコホート研究において、抗菌薬投与遅延を予防するための標的となりうる典型的な慣例が関係する因子を特定した。抗菌薬投与前に脳脊髄液または尿サンプルを採取することや、既存の投与経路がない患者において、末梢静脈カテーテルを挿入するのにより長い時間がかかること、輸液投与により長い時間がかかることは、抗菌薬投与までの時間が中央値よりも長いことに関連していた。

カナダの 3 次 NICU 2 施設からの報告。遅発性血流感染症の早産児において早期の抗菌薬投与は予後改善に重要であり、検体採取や血管確保に時間がかかることが抗菌薬投与の遅延につながることが理解できる。新生児領域に関わる方には特におすすめする。
