背景

多剤耐性(MDR)グラム陰性細菌(GNB)感染症は、依然として手術患者における罹患および死亡の重要な原因である。本研究の目的は、腹部手術後の患者における MDR-GNB 感染症の危険因子を特定すること、ならびに死亡と独立して関連する予測因子を明らかにすることであった。

 

方法

2010 年から 2017 年に、外科集中治療室(ICU)に入室した腹部手術患者を対象に後向きコホート研究を実施した。GNB 感染症患者を解析対象とした。

 

結果

合計で、364 例の患者が腹部手術後に GNB 感染症を発症した。これらの患者のうち 117 例(32.1%)が MDR-GNB 感染症であった。MDR-GNB 分離株 133 株中、最も多かった分離株は大腸菌(Escherichia coli)(45.1%)であった。MDR-GNB 感染症患者では、非 MDR-GNB 患者と比較して、人工呼吸器装着期間および入院期間がより長く、また 30 日死亡率および院内死亡率がより高かった。多変量解析から、ICU 入室前の期間がより長いこと、外科的再検索、group 2 カルバペネム系薬*(例、イミペネム、メロペネム、ドリペネム)およびフルオロキノロン系薬の使用、ならびに総ビリルビン高値が、MDR-GNB 感染獲得の独立したリスク因子であることが示された。MDR-GNB 感染症患者における30 日死亡の予測因子は、慢性腎臓病、group 2 カルバペネム系薬*の使用、および不適切な経験的抗菌薬療法であった。

 

結論

本研究は、腹部手術後の患者における MDR-GNB 感染症および 30 日死亡のリスク因子に関する重要な情報を提供している。
 

* (監訳者注)group 2 カルバペネム系薬:ブドウ糖非発酵菌に抗菌活性のある広域スペクトル薬で、日本で一般的に使用されているカルバペネム系薬である。(日本では group 1 カルバペネム系薬であるエルタペネムは未承認である。)

監訳者コメント

台湾からの報告で、多剤耐性グラム陰性細菌(MDR-GNB)の多くが ESBL 産生菌で日本の多剤耐性率と比べるとやや高めだが、大差はない。結果は腹部手術後で一般的に言われている危険因子になるが、日本でも参考になる。
