A user’s view of commercial mobile applications designed to measure hand hygiene compliance by direct observation

背景

直接観察による手指衛生遵守率の測定を容易にするモバイルアプリケーションは広く普及している。感染予防・制御（IPC）の専門家にとってそれらの有用性は、レビューも、正式な評価も近年なされていない。

目的

手指衛生評価アプリケーションの厳密な分析を提示すること。

方法

PubMed、Apple アプリストア、Google Play アプリストア、Google 検索エンジンの 4 つの情報源からモバイルアプリケーションを特定した。次いで、IPC 専門家にとって重要とされる 7 つの主要基準を用いた新規評価システムに対して、個々のアプリケーションを評価した。この主要基準は、有用性、価格、自動データ分析、研修の要件、世界保健機関（WHO）の手指衛生の 5 つのタイミングにおいて評価される遵守、最新情報、アプリストアの平均評価であった。各基準について可能性があるスコアは 0 ～ 2 とし、個々のアプリケーションにつき得られる最大スコアは 14 とした。

結果

計 32 個のアプリを特定し、そのうち 13 個のアプリが分析に適していた。12 点以上が得られたアプリは 3 個（19％）のみであった。WHO の手指衛生の 5 つのタイミングにおいて遵守が評価できたアプリは、12 個（92％）であった。無料で使用できたアプリは 5 個（38％）、自動分析と手指衛生データの報告が可能であったアプリは 7 個（54％）。アプリストアで高評価が得られたアプリは 6 個（46％）だけであった。

結論

手指衛生の直接観察に利用できる現行のモバイルアプリケーションは、概してスコアが不良であった。これらのアプリがIPC 専門家にとってより有用となるように、使い易く、使用前の研修が少なくて済むものにする必要がある。今後の開発を導くために、IPC の全国組織は、このようなアプリの中核的な基準を作成すべきである。

監訳者コメント：

手指衛生モニタリングのアプリは、観察記録がそのままデータとして保存され解析されることから大変有用であり、すでに導入している医療機関も少なくない。論文本文には具体的なアプリ名とその評価が記載されており、使用する際の参考になると思われた。
