Impact of a surgical safety checklist on surgical site infections, antimicrobial resistance, antimicrobial consumption, costs and mortality

背景

世界保健機関（WHO）の手術安全チェックリストについての勧告後、2010 年に当院ではこのチェックリストを中心とする手術安全プログラムを実施した。

目的

本研究の目的は、このプログラムの実施前（2006 年 1 月から 2010 年 7 月）と実施後（2010 年 8 月から 2014 年 12 月）の、手術部位感染症、抗菌薬使用量、抗菌薬耐性、費用、および院内死亡率の指標を比較することである。

方法

介入の影響を調べるために、手術部位感染症（SSI）発症患者と SSI 非発症患者とのマッチングによる症例対照研究を実施した。

結果

手術安全チェックリストの使用は、SSI の有意な減少と関連した。2 つの期間を比較した場合、ESKAPE グループに属する病原菌による感染症の減少（90.7％から 73.9％に減少、P ＜ 0.001）、汚染創、感染創、汚染の可能性のある創を有する患者の SSI 減少、周術期の予防的抗菌薬が 48 時間未満で中止された患者における SSI 減少も確認された。全体として抗菌薬耐性の低減がみられたが、エンテロコッカス・フェシウム（Enterococcus faecium）のカルバペネム系耐性およびグリコペプチド系耐性、黄色ブドウ球菌（Staphylococcus aureus）のクリンダマイシン耐性は増加した。第二世代および第三世代セファロスポリン系、ならびにクリンダマイシンの抗菌薬使用量の増加も確認された。院内死亡は 6.4％から 3.2％（P ＝0.001）に減少したが、費用の削減は認められなかった。

結論

手術安全チェックリストの実施は、SSI 低減の独立予測因子であり、さらに抗菌薬耐性の減少および院内死亡率の低下と関連した。
