背景

血液腫瘍患者は家庭および病院の給水システムの両方でよく見られるマイコバクテリウム・ケロナエ(Mycobacterium chelonae)のような非定型抗酸菌による感染症のリスクが高い。

 

目的

研究対象病院における M. chelonae の患者 2 症例および陽性の水サンプルに続く調査および制御策について述べること。

 

方法

放水口、貯蔵タンクおよび給水本管の水質検査を行った。全ゲノムシークエンシング(WGS)を用いて患者と陽性の水サンプルを比較した。その後に実行した感染制御策について述べた。

 

結果

WGS の結果は採取された分離株群における M. chelonae の 2 つの主要な集団を示した。本結果は患者株が互いに無関係であることを示したが、患者 1 例由来の分離株は放水口由来の環境サンプルと近縁関係にあり、院内での獲得を裏付けた。

 

結論

WGS を用いて M. chelonae の 患者 2 症例と病院の給水システム由来の陽性の水サンプルを調査した。関連する制御策と、給水システムの薬品注入が非定型抗酸菌の増殖を促進させる可能性について考察した。

監訳者コメント:

病院の水質検査で得た 147 検体中 68 検体からM. chelonaeが検出され、そのうち 34 検体は 100 CFU/mL 以上の菌量だった。筆者らは、院内発症の非結核性抗酸菌症患者が 1 例でも発生したらまず病院の水を疑えと結論している。日本でも最近の検討では公衆浴場や下水の水の約 30%から非結核性抗酸菌が検出されている。非結核性抗酸菌は一般細菌検査では見逃す可能性があり、検出されないからといって存在しないと決めつけることはできない。筆者らの主張のとおり、院内発症事例があった場合は特に注意が必要である。
