背景

Public Health England の指針では、エアロゾル発生手技に携わる医療従事者にFFP3マスクの使用を規定している。呼吸器防護具のマスクフィットテストは、医療従事者をエアロゾル粒子から保護するために不可欠である。

目的

新型コロナウイルス感染症(COVID-19)パンデミックの第1波での、英国の国民保健サービス(NHS)病院におけるマスクフィットテストの成績について分析すること。

方法

2020 年 5 月 26 日に情報公開法(Freedom of Information Act)を用いて、独立した研究者 1 名がNHS 病院 137 施設に連絡して、各施設におけるフィットテストの成績に関するデータを報告するよう要請した。

結果

2020 年 5 月 26 日から 2020 年 10 月 29 日に、病院 96 施設が要請に対して回答した。病院 56 施設において計 86 種類のマスクが使用されており、このうち 13 種類がこれらの病院の 10%以上で使用されていた。最も多く用いられていたのはFFP3M1863 で、病院の 92.86%で使用されていた。全体で、フィットテストの合格率が病院 32 施設から報告され、平均合格率は 80.74%であった。フィットテスト不合格率からみて最も成績の良かったマスクは、Alpha Solway 3030V およびAlpha Solway S-3V(いずれもフィットテストの平均不合格率 2%)であった。男女別の合格率および不合格率が、病院 7 施設から報告された。これら 7 施設において、男性の 20.1%が用いられたすべてのマスクについてフィットテスト不合格であった一方、女性では 19.9%が用いられたすべてのマスクについてフィットテスト不合格であった。不合格率は、黒人、アジア人および少数民族群(BAME)のスタッフで有意に高く、病院 4 施設において 2,507 名中 644 名(25.69%)であった。

結論

パンデミック第 1 波に対する対応時にテストを受けた医療従事者の 20%が、各種マスクのフィットテストで不合格であった。少数サンプルから、この結果は BAME 群のスタッフで最も顕著であることが示された。

監訳者コメント

英国での COVID-19 パンデミック初期での FFP3(N95)マスクのフィットテストで約 20%で不合格となった。黒人、アジア人および少数民族群で、不合格率が有意に高かった原因としては民族性による顔の形の違いが挙げられる。
