背景

組織バンク業務において、微生物汚染は制御しなければならない主要なリスクの 1 つである。したがって、厳密なドナー選択基準の適用、消毒プロトコールの利用、微生物モニタリングが諸段階で実施される。

目的

動脈同種移植片のカンジダ・オーリス（Candida auris）を検出し、その原因を評価すること。

方法

ドナー 2 例より複数の組織が提供され、微生物培養で C. auris 陽性が認められたので、これらのデータに関して分析した。調達、処理、保存後の時点で、微生物汚染のリスク因子を評価した。

結果

培養で C. auris が分離されたのは動脈のみで、角膜、筋骨格組織、皮膚からは培養で検出されなかった（1 例のドナーから採取された腋窩および直腸検体においても）。

結論

C. auris による動脈汚染を説明する上でもっとも可能性のある原因として、ドナー自身の皮膚が特定された。この真菌の病原性および正確な同定に関連する問題のため、組織提供に際して C. auris の検出を図るための施策が推奨される。

監訳者コメント：

ドナーから提供された動脈からカンジダ　オーリスが検出されたから、組織培養を行った。採取手技の逸脱などはなく、ドナー自身の皮膚が感染源と考えられたという論文である。微生物検査を含む疫学調査からリスク評価を行い改善策につなげていくプロセスは大変重要だと思われた。
