Measurement and interpretation of hand hygiene compliance rates: importance of monitoring entire care episodes

この研究の目的は、手指衛生遵守率を測定・解釈するため、患者および周辺環境との一連の連続的な接触機会における手指衛生の遵守をモニターすることの重要性の評価である。集中治療室（ICU）4施設と非集中治療病棟（NICW）4施設を対象として、手指衛生遵守の直接的観察研究を実施した。手指衛生の機会を2つのカテゴリー、すなわち、「一連の接触機会外（接触が1回の場合はその前または後、一連の接触機会が連続した場合は最初の接触前または最後の接触後）」、および「一連の接触機会中（一連の接触機会の、最初の接触後から最後の接触前まで）」に分類した。全体の手指衛生機会は ICU 903回、NICW 760回であった。「一連の接触機会中」の比率は、ICU 46.0％、NICW 22.9％であった。全体の手指衛生遵守率はNICWのほうがICUよりも有意に高かった（61.2％対47.5％ 、P < 0.00001）。「一連の接触機会外」の手指衛生遵守率は「一連の接触機会中」より有意に高かった（67.7％対28.5％ 、P < 0.00001）。「一連の接触機会中」の手指衛生遵守率は、ICU のほうが有意に高かった（32.2％対19.0％、P < 0.005）。NICWで観察された手指衛生機会のカテゴリーの比率がICUと同一であったと仮定すると、全体の手指衛生遵守率はNICW（46.4％）、ICU（47.5％）と同等になると考えられる。全体的な遵守率の著しい過大評価を回避するために、一連の連続的な接触機会におけるケアエピソード全体の手指衛生遵守をモニタリングする必要がある。同時に、遵守のデータを比較する際は、研究の評価対象とする「一連の接触機会中」の比率を考慮すべきである。

実際のケアプロセスは「一連の」流れの中で提供されることから、手指衛生の実践遵守についても「一連の接触機会中」における評価が重要であるというのは理解できるが、実際に現場で手指衛生遵守の状況を評価するのは極めて困難であると認めざるを得ないところか？
