Hand-hygiene-related clinical trials reported between 2014 and 2020: a comprehensive systematic review

背景

医療関連感染症の予防のために手指衛生はもっとも有効な手段であるという一般的な合意が得られている。しかしながら、医療従事者における遵守率が低いことが世界的に報告されている。新型コロナウイルス感染症（COVID-19）のパンデミックにより、医療従事者の手指衛生遵守の全体的な改善を図る必要性がさらに重視されている。

目的

今回の包括的なシステマティックレビューは、2014 年から 2020 年に報告された臨床試験の最新の編集物を提供するものであり、手指衛生により医療関連感染症の低減を図るアプローチに関して医療指導者および実践者に情報を提供するために手指衛生介入を評価する。

方法

CINAHL、Cochrane、EMbase、Medline、PubMed、Web of Science のデータベースを用いて、医療従事者の手指衛生遵守に関する主題で 2014 年 3 月から 2020 年 12 月に発表された臨床試験を検索した。この検索により計 332 報が特定され、このうち 57 報が選択基準を満たした。

結果

57 報のうち本レビューの対象とした 45 報（79％）が、アジア、欧州、米国で実施された。これらの臨床試験の大多数は、病棟および集中治療室を含む急性期病院で実施された。試験対象の医療従事者のうち看護師がもっとも多く（44 報、77％）、次いで、医師であった（41 報、72％）。36 報（63％）は世界保健機関（WHO）の多角的フレームワークまたはこのフレームワークを変化させたものを採用し、その多くが「5 つのタイミング」の各時点での手指衛生の機会を記録した。しかし、手指衛生手技の記録はあまりみられなかった。

結論

医療従事者の手指衛生遵守を図る単独介入と多角的な手指衛生対策はいずれも、軽度から中程度の改善をもたらしうる。
