Evaluation of procalcitonin as a contribution to antimicrobial stewardship in SARS-CoV-2 infection: a retrospective cohort study

新型コロナウイルス感染症患者において抗菌薬の使用を安全に差し控えることができるかの判断は、診断上の課題である。本研究では、国民保健サービス（NHS）病院トラストシェフィールド教育病院で採用されている、血清プロカルシトニン（PCT）低値（0.25 ng/mL以下と定義）の患者では抗菌薬を差し控えることを推奨するガイドラインの有効性を評価した。その結果、PCT ≦ 0.25 ng/mL の患者において抗菌薬の使用を減らしても死亡件数の増加はみられないことが示され、あわせて入院中にその後のカルバペネム処方が減少した。これらの結果は、同ガイドラインの有効性を支持するものであり、これらの患者における最適な PCT のカットオフ値を明らかにするためにさらなる研究を行うことが勧められる。

新型コロナウイルス感染症に特効薬はいまだに存在しないが、標準的な治療法は確立しつつある。未知の感染症に対しては適切な治療がないがしろにされがちであるが、原点に立ち返り、抗菌薬適正使用も考慮すべきである。PCT を活用した抗菌薬適正使用はコロナ前より言われているがひとつの選択肢となる。
