背景

手術部位感染症は世界的な患者の安全に関する懸念の 1 つである。汚染に関するエビデンス不足のため、前もってセットされた手術関連の物品は処分されるときもあれば再滅菌されるときもあり、それゆえコスト、資源の利用および環境への影響を増大させている。

 

目的

手術室におけるカバーありおよびカバーなしの滅菌された物品に関し、経時的な空気中の細菌による汚染を検討すること。

 

方法

血液寒天培地(N = 1,584)を用いて滅菌野の空気中の細菌による汚染を 48 回検出した。毎回、3 つの好気性菌培地および 3 つの嫌気性菌培地を準備時間のモデルとなるベースラインとして用い、60 の培地(好気性菌培地 30 および嫌気性菌培地 30)を手術前の待機時間のモデルとして用いた。これらの半数は滅菌ドレープでカバーされ、残りの半分はカバーをしないままであった。4、8、12、16、24 時間後に培地を収集した。

 

結果

汚染前の平均時間はカバーなしの群では 2.8 時間(95%信頼区間[CI]2.1 ~ 3.4)、カバーありの群では 3.8 時間(95%CI 3.2 ~ 4.4)であった(P = 0.005)。カバーなしの群では 98 コロニー形成単位(cfu)であったのに対し、カバーありの群では 20 cfu であった(P = 0.0001)。16 の異なる微生物が分離され、最もよくみられたのはキューティバクテリウム・アクネス(Cutibacterium acnes)であり、次いでミクロコッカス・ルテウス(Micrococcus luteus)であった。ブドウ球菌(Staphylococcus)32 cfu のうち 14 cfu は抗菌薬耐性であり、多剤耐性表皮ブドウ球菌(Staphylococcus epidermidis)1 cfu を含んだ。

 

結論

滅菌カバーを用いて滅菌野を空気中の細菌による汚染から保護することで、滅菌された物品の耐久性を 24 時間まで延長できる。外科用滅菌器具は急性期手術のため事前に準備される可能性があるので、滅菌された物品の耐久性の延長は患者安全に役立ち、それによって治療の質を向上させ、気候変動による影響とコストの両方を減少させられるかもしれない。

監訳者コメント

手術室において培地を滅菌ドレープでカバーする群としない群に分け、細菌汚染を比較した研究。単純な研究だが直感的で面白い。
