Knowledge, perceptions and experiences of nurses in antimicrobial optimization or stewardship in the intensive care unit

抗菌薬耐性に対処する包括的行動を支援するために、専門家間の共同アプローチの差し迫った必要性が認識されている。抗菌薬適正使用支援は、医療機関内の抗菌薬最適化のための系統的アプローチを参照する。集中治療室（ICU）など抗菌薬使用率が高い領域において、抗菌薬適正使用支援に関して看護師の具体的な役割が明確に定められていない。今回のレビューの目的は、ICU における抗菌薬の使用および最適化と関連する看護師の知識・認識・経験について調査した主要研究を特定し、厳密に評価することである。Medline、CINAHL、PsychINFO、EMBASE、PubMed、SCOPUS、Cochrane Library、Web of Science のデータベースで、2000 年 1 月 1 日から 2020 年 3 月 20 日に発表された主要研究の系統的検索を実施した。定量的および定性的データを統合するために収束的統合デザインを用いた。最初にスクリーニングした研究 898 報のうち 26 報を対象とした。大半（26 報中 18 報）が定量的研究であった。定性的研究（26 報中 6報）のすべてが方法論的質の高いものであった。介入が用いられた研究（26 報中 10報）では、ICU 看護師が抗菌薬使用・抗菌薬投与までの時間・過誤率を低減させる可能性が大きいことが確認された。看護関与を阻む障壁は、抗菌薬使用に関する知識不足、専門家間の非協調性、医師に従う習慣などであった。抗菌薬使用の最適化にあたり看護師の役割を強化する要因として、教育の向上、技術利用、有能な看護指導力、看護師を支援する堅実な組織構造が確認された。今回のレビューでは、看護イニシアチブにより ICU の抗菌薬最適化が強化される可能性がかなり高いことが示された。積極的な関与を阻む障壁と実現させる要因が特定された。

ベッドサイドで患者の病態を頻回に確認している看護師こそ、適切な抗菌薬処方の支援チームに必要な一員であり看護師による個々の患者の情報を基にした抗菌薬の専門家による診療支援と相まって最適な抗菌薬療法が実施可能となる。
