院内 COVID-19 は 30 日死亡率を高めるか？ COVID-19 患者における転帰不良のリスク因子を明らかにするための多施設共同観察研究★

本研究の目的は、院内感染新型コロナウイルス感染症（COVID-19）では、市中獲得 COVID-19 よりも転帰が不良であるかどうかを明らかにすることであった。本研究は、2020 年 4 月 9 日時点で、急性期病院 3 施設においてCOVID-19 が確認されたすべての入院患者を対象とした前向きコホート研究であった。患者の追跡調査を 30 日以上実施した。院内感染は、入院後 7 日目におけるスワブ検査陽性と定義した。全体で、患者 173 例が特定され、19 例（11.0％）は院内感染であった。スワブ検査陽性患者 32 例（18.5％）が 30 日以内に死亡（全死因）し、3 群間（すなわち、COVID-19 疑いの入院患者、COVID-19 を発症した患者、および院内感染 COVID-19 の患者）で 30 日全死因死亡率に有意差はなかった：21.1％ 対 17.6％ 対 21.6％（P ＝0.755）。院内感染 COVID-19 には、市中獲得 COVID-19 と比べて、死亡率の上昇は認められなかった。
監訳者コメント：
COVID-19 は市中感染よりも院内感染の方が死亡率が高そうに考えられるが、実際は有意差がなかったというスコットランドからの報告。他にも院内感染 COVID-19 の方が死亡率が低いとの英国からの報告（Journal of Hospital Infection (2020) 106, 376-384）があり参照されたい。
