Genomic analysis revealed distinct transmission clusters of vancomycin-resistant Enterococcus faecium ST80 in Stockholm, Sweden

シークエンス型（ST）80 に属するバンコマイシン耐性エンテロコッカス・フェシウム（vancomycin-resistant Enterococcus faecium；VREfm）は、この 3 年間にストックホルムにおける優勢なクローン系列となった。ST80 は 2018 年には VRE 症例の75％を、2019 年には 46％ を占め、vanA 型と vanB 型の両方のアウトブレイクを引き起こした。重複のない ST80-VREfm 分離株（N ＝ 188）について全ゲノムシークエンシングを行った。ゲノム解析によって 3 つの別々の伝播クラスターが明らかになった。我々の研究によって、表現型検査によるバンコマイシン軽度耐性分離株の検出の難しさが、長期間の vanB 型のアウトブレイクの説明要因の 1 つである可能性が示された。本稿ではストックホルムにおける最初の optrA 陽性リネゾリド耐性VRE 分離株についても報告する。

監訳者コメント：

スウェーデンの首都ストックホルムにおける VRE の地域アウトブレイクについての論文であるが、vanB 型の VRE は VCM の感受性が 4μg/mL から 256μg/mL と広範囲であり、軽度耐性の場合は通常の薬剤感受性検査で見逃す可能性が高く、同一病棟での複数例で腸球菌が分離された場合には vanB 型 VRE の可能性も考えておく必要がある。また、スウェーデンでの第 1 例目となったリネゾリド耐性 vanA 型 VRE は、82 歳の透析患者から分離されており、幸運にも感染拡大は認められなかった。optrA 遺伝子はリネゾリドを含むオキサリジノン系抗菌薬への耐性を示し、近年 EU でのリネゾリド耐性腸球菌の報告が増加しており、日本においてもすでに同様の耐性を獲得した E. faecalis がすでに報告されており警戒が必要である。
