Acquired resistance to chlorhexidine – is it time to establish an ‘antiseptic stewardship’ initiative?

クロルヘキシジングルコン酸塩(CHG)は、手指衛生、皮膚消毒、口腔ケア、患者の清拭などにおけるさまざまな適用で使用される抗菌薬である。使用の増加は、細菌の耐性獲得発現の懸念をもたらす。CHG の臨床分離株に対する最小発育阻止濃度(MIC)を含む公表データを検討し、耐性を決定する疫学的カットオフ値と比較した。CHG 耐性は、大腸菌(Escherichia coliM)、サルモネラ(Salmonella)属菌、黄色ブドウ球菌(Staphylococcus aureus)、コアグラーゼ陰性ブドウ球菌ではめったに認められない。しかし、エンテロバクター(Enterobacter)属菌、シュードモナス(Pseudomonas)属菌、プロテウス(Proteus)属菌、プロビデンシア(Providencia)属菌、エンテロコッカス(Enterococcus)属菌では、分離株の CHG 耐性がしばしば認められる。CHG 耐性は、超多剤耐性肺炎桿菌(Klebsiella pneumoniae)などの多剤耐性分離株で検出される場合がある。MIC が高い分離株ほど消毒用 CHG 感受性が低いことが多い。抗菌薬との交差耐性は依然として議論が分かれているが、いくつかの研究から、CHG への全面的な曝露が一部の抗菌薬に対する耐性リスクを増加させることが示唆される。CHG 耐性は多くのアウトブレイクや医療関連感染の原因となっている。平均的な集中治療室の場合、液体石けんや擦式アルコール製剤に CHG が含まれるなら、CHG 曝露の大半はそうした手指衛生製剤によるものと説明されるだろう。致死量未満の濃度の CHG 曝露は、いずれも新興の、抗菌薬耐性がよく知られた種であるアシネトバクター(Acinetobacter)属菌、肺炎桿菌、Pseudomonas 属菌の耐性を増強する可能性がある。院内病原菌へのさらなる選択圧を低下させるために、高価な薬剤である CHG を患者利益の明らかな適応症に限定し、いかなる利益もない、あるいは利益が疑わしいものへの適用は除外することが意味のあることと思われる。

監訳者コメント
抗菌薬耐性と消毒薬抵抗性は同一遺伝子で規定されているわけではないが、こうした薬剤が多用されている医療環境では、微生物が抗菌薬に対しても消毒薬に対してもそれぞれの多用により選択圧がかかるため高度耐性(抵抗性)菌の分離比率が高くなると考えられる。
